Insurance Coverage and Claims
Intercollegiate Athletic Insurance:
Athletic Insurance is provided by Concordia College for the benefit of our intercollegiate student-athletes. For the 1999-2000 school year, Athletic Insurance coverage is provided by ReliaStar Life Insurance Company and administered by Student Assurance Services. This coverage is offered on an "excess" basis only, which means ReliaStar Life Insurance Company will only cover expenses after the athlete's primary carrier has been used to its fullest extent. Primary coverage would include parental insurance under which the student-athlete is covered as an eligible dependent.
The deductible for this athletic insurance policy is $500 per injury. If the student-athlete is injured in a separate accident at a later date, he or she will be required to pay another $500 deductible before ReliaStar Life Insurance Company will begin covering any expenses.
ReliaStar Life Insurance Company will pay up to $50,000 for maximum medical benefit and will pay up to $ 10,000 for Accidental Death and Dismemberment.
The policy also covers the student-athlete while traveling directly to or from such practice or competition in a vehicle designated and supervised by Concordia College. Coverage is only in effect during the official season of the sport in which the student-athlete is a participant.
It should be noted that all students at Concordia have the option of purchasing health insurance under the Student Health Plan. However, the Student Health Plan will not cover any injury sustained while a student is practicing or playing in intercollegiate sports (this is a specific exclusion under the Student Health Plan).
Claims:
When a student-athlete is injured (not including minor scrapes and
bruises), he or she must request a claim form from the head athletic trainer
in the Athletic Department. The student must complete and sign the front
page of the claim form and return it immediately to the trainer. Concordia
College will then forward the claim form to Student Assurance Services
(they require notification with 30 days of the injury). When the student
picks up the claim form, the head athletic trainer will also provide them
with a handout that explains the claims process in detail. Dawn Current

Catastrophic Injury Insurance:
In addition to Athletic Insurance, Concordia College also provides
all student-athletes with Catastrophic Injury Insurance. This plan is sponsored
by the NCAA and covers catastrophic injuries such as:
Spinal cord injury
Severe head/neck trauma
Partial or complete paralysis
Injuries resulting in severely diminished mental capacity or other
neurological injury
Irrecoverable loss of speech, hearing (both ears), sight (both eyes),
both arms, both legs, or one arm/leg
The policy has a $50,000 deductible and provides benefits in excess of any other valid and collectible insurance.

"CLAIMS PROCESS"
FOR INTERCOLLEGIATE ATHLETIC
INSURANCE
Concordia College has insurance coverage through Student Assurance Services, Inc. (SAS) to cover athletic related injuries that result in medical expenses IN EXCESS OF $500. This coverage is for intercollegiate athletes only, not for intramural or club sports participants.
If you have your own medical insurance coverage or if you are covered under your parent's medical plan, then all your injury related claims should be submitted to your insurance carrier first. However, even if you have other insurance, you should immediately file a claim form with Student Assurance Services Inc. also.
Below is a list of step by step instructions to file an athletic injury claim with SAS:
1. Request a claim form from the head athletic trainer, Athletic Department. Please answer ALL questions, sign, and return to the trainer. NOTE: Be sure to state the correct date of injury and use that date consistently with all providers you see with regard to your injury. SAS requires notice of an injury within 30 days of its occurrence.
2. The athletic trainer will forward your completed claim form to Dawn Current, Business Office, for processing directly with the insurance company. At this point, we will be sending in just the claim form.. .no itemized bills, etc.
IF YOU HAVE OTHER MEDICAL INSURANCE COVERAGE, SEE ITEM 3. IF NOT, SKIP ITEM 3 AND GO DIRECTLY TO ITEM 4.
3. IF YOU HAVE OTHER INSURANCE COVERAGE, submit all your injury related medical bills to your insurance FIRST! If the total of all your bills is less than $500, no further action is necessary. However, if the total is at least $500, please bring all itemized statements (not balance due statements) along with the Explanation of Benefits (EOB's) from your insurance company TO THE BUSINESS OFFICE. We will then submit the remaining balance/s to SAS for payment consideration.
4. IF YOU DO NOT HAVE OTHER INSURANCE and the total of your injury related medical bills is at least $500, then bring all your itemized statements (not balance due bills) TO THE BUSINESS OFFICE and we will submit them to SAS for payment consideration. NOTE: The first $500 of these bills will be your responsibility.
